Comprehensive Clinical Pharmacist Series: Cardiovascular Disease

This certificate program provides the essentials for clinical management of cardiovascular disease and diabetes by licensed pharmacists. In addition to providing a strong knowledge base of disease pathology and clinical manifestation, the most recent clinical guidelines, and pharmacotherapeutic recommendations, the P3 Process of Care provides a proven framework for pharmacist as they engage in patient- centered Comprehensive Medication Therapy Management in any confidential counseling setting. Pharmacists completing this certificate will be equipped to provide care for patients with cardiovascular disease and diabetes, and will have the assessments and patient education tools to enroll patients upon completion.
