The P3 Process of Care: Comprehensive Medication Therapy Management (CMTM)

The P3 Program™ Process of Care is an evidence-based therapeutic process delivered by trained pharmacists for the management of patients with chronic disease(s). This process is called Comprehensive Medication Therapy Management (CMTM) services, which is an interactive P3 clinical service guided by a propriety health information technology, ThinkEHRx™. P3 Program™ CMTM services have been studied and proven to improve the health status of patients while reducing unnecessary costs of care.
Learning Objectives
- Apply the six-step P3 Program™ model of care for the management of patients with chronic diseases; and
- Identify resources and tools to conduct comprehensive medication therapy management.
